Care Management

What is Care Management

Care Management supports the department of Student Support and Community Standards by identifying immediate needs, connecting to appropriate resources, and working to help students navigate challenges to support their well-being and success. Care Management responds to UCARE referrals and connects students with support and resources.

Care Managers individually assess each student’s needs and explore resources on campus and within the community to facilitate connections to the most appropriate support. Because a successful plan may include multiple resources, Care Managers can provide continuity of care and accountability through supportive follow-up meetings with students to evaluate successes, barriers, and/or gaps in support.


Confidentiality Notice: The Care Manager is not bound by the same level of confidentiality as the counseling staff. They are a mandated reporter but can refer you to a confidential source.
